Job Summary for RNAC:
The primary function of this position is to conduct and coordinate the development and accurate completion of the resident assessment and care plan process, as well as develop, implement, monitor and supervise the restorative program of the facility. Candidate will also assist with Discharge Planning.
Some job duties include, but are not limited to:
- Conduct and coordinate the development and accurate completion of the resident assessment and care planning process, including the implementation of CAA's.
- Ensures that quarterly, annual, change of condition, etc., resident assessments and care plan reviews are conducted in accordance with regulatory guidelines.
- Assists facility directors and supervisors in scheduling the resident assessments and care plan meeting.
- Develop and participate in planning and conducting in-service training, which includes assessment skills, techniques to complete the assessment tool and other areas as directed.
- Actively participates in overall facility management and makes recommendations for performance improvement, i.e., chart audits.
- Completes all Third Party payor forms as necessary, i.e., MIF, MA 103, etc.
