Position: Case Manager 2
Program: Cornerstone
Location: New Haven
Salary: $21.00/hour
Schedule: Full time, 40 hours, Monday-Friday, 2nd shift (2:30-10:30p)
Program Summary:
A transitional group home providing psychosocial rehabilitation to adults with serious and persistent mental illness, who have a history of psychiatric hospitalization and impaired social functioning, in order to reduce the frequency of hospitalization and improve skills related to self-care and independent functioning in the community.
Position Summary:
This position will be responsible for providing case management services to a specified case load of residents, supportive counseling, psycho-social activities and coordinating resident services. The Case Manager 2 functions as a team member of the program and as a member of designated resident treatment team. This position is responsible for carrying a caseload, for implementing and facilitating resident activities/interventions, and for providing program shift coverage.
Working within Adult Services, the Case Manager 2 works on the Behavioral Health Treatment care team to support individuals with severe and persistent mental illness residing in a residential setting.
Requirements:
- High School Diploma or GED
- 3 years of relevant experience
- Valid Driver’s License
- Regular and predictable attendance is required
Orientation:
If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
