Schedule: Full time, 40 hours, 1st shift, Monday-Friday
Program Summary:
The Start Program is a nationally recognized program that works with homeless and unstably housed youth 18 – 24, located throughout the state of Connecticut through an innovating service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.
The Start Crisis Emergency Housing Program is a 16-bed program specifically for homeless youth adults ranging in age 18 -24 in the Greater Hartford area. The Program works with youth who are referred via CAN, verified homeless and on the waitlist for crisis housing to ensure that they quickly exit homelessness and into stable housing.
Knowledge, Skills and Position Summary:
The eligible candidate for this position will work full time, 40 hours, 2nd shift, Sunday (9-5p), Monday through Thursday (12-8 p) with occasional weekend coverage responsibilities, holiday, and on-call availability to fit program needs.
The Case Manager 2 – Crisis Housing worker is responsible for providing staff support and direct client care to homeless youth age 18 to 24. These youth are in the Greater Hartford area transitioning from crisis housing to possibly a youth housing program and/or independent living in the community. This position is non-exempt and works under the supervision of program management.
Staff responsibilities include but not limited to:
- staffing the program’s youth specific agency leased crisis housing apartments
- helping clients to seek/maintain employment,
- become document ready (obtaining State ID, birth certificate, SS card etc.
- Assist clients in applying and/or helping them to determine their eligibility for SNAP, SSI, Husky insurance via application with appropriate State agency for benefits.
- conduct intake assessments, identifying and engaging homeless youth in the community,
- engaging other community providers to help identify homeless and at-risk youth in the community,
- providing survival aid supplies,
- working with youth to exit homelessness,
- delivering comprehensive case management services to clients, transportation to appointments/resources as needed,
- development and implementation of individual care plans
- is a liaison with treatment providers and community resources,
- coordination with local educational/vocational resources.
Education:
The minimum qualifications to be an eligible candidate for the Case Manager 2 position is:
- An Associate degree and 2 years of related experience in the direct service and case management field
- Valid Connecticut driver’s license
Orientation:
If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
