Description
This position will primarily be responsible for development and implementation of all therapeutic strategies in accordance with the Family based model, providing guidance and structure to actively support the child and/or families in achieving their treatment plan goals.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Provide a minimum of family and/or individual therapy weekly for each assigned family while providing ongoing case management, crisis and/or social intervention.
- Provide a minimum of 25 hours of billable units including a minimum of 10 hours of individual and/or family therapy and 15 hours of team therapeutic/case management services on a weekly basis
- Works with team partner 60% of the time for family sessions and crisis intervention.
- Develops and maintains complete clinical records including assessments, treatment plans, progress notes, case conferences, discharges and other clinical and administrative documents according to program standards for timeliness. Ensures charts adhere to established chart sequence, date order, and that all documents are present.
- Ensures that treatment plans are completed every 30 days for each family. Timely submission of clinical developed case documentation including assessments, progress notes, treatment plans, case conferences and discharge summaries
- Completes assessments, treatment plans, progress notes, discharges and other clinical and administrative documents in accordance with agency, managed care and licensing requirements and standards.
- Regularly checks to ensure that content, signatures, credentials, dates, times,service types and legibility meet standards.
- Identifies community resources and facilitates linkages with psychiatric, social, vocational, medical, housing, recreational, forensic, and drug/alcohol systems for the coordination of services to meet clients’ need.
- No less than 25% of weekly work time in the office completing paper work, attending bi-weekly staff meetings and weekly supervision and monthly case conferences as scheduled. Ability to provide a thorough review of current caseload including a progress report and proactive action plans to address areas of concerns.

Education and/or Experience
Bachelors degree or equivalent; three to five years related experience and/or training; or equivalent combination of education and experience.
