Job Details
Purpose:
- This individual will provide administrative/clinical leadership and supervision to the Supports Coordinators on their team. Coordinates the activity of the team and assists the Director and Manager as per assignment.
Supervision Received:
- This individual performs duties independently reporting directly to the Unit Manager
Supervision Exercised:
- Supervises Supports Coordinators within the unit
Trains, mentors and evaluates the Supports Coordinators of their team. |
Supervises the Supports Coordinators in the daily activities and schedules related to their individual caseload assignments. Assures that monitorings, IM4Q responses, service notes, and ISPs are completed within necessary time frames. |
Reviews and submits consumer annual Individual Service Plans in HCSIS. Facilitates the implementation and evaluates the effectiveness of the plans for the Supports Coordinators on their team. |
Oversees the maintenance of supports coordinators caseloads records & ensures paperwork is consistent with agency, regulatory, HCSIS & funding requirements. Reviews Daily Activity Logs for consistency with HCSIS. Ensures that HCSIS service notes are entered within two days of the service provision. |
Attends County meetings/committees as a representative of the Unit, as needed. |
Establishes and maintains consumer friendly environment in the office. Creates spirit of customer service throughout all Supports Coordination Activities. |
Monitors staff schedules, attendance and approves time-off. |
Tracks in-house training for Supports Coordinators on team and ensure that their training is up-to-date. |
Continually develops team knowledge & cooperation. |
Identifies and compiles reported gaps in the service delivery system and reports them to Management Team. |
Performs necessary functions in the state HCSIS system. |
Perform other duties and responsibilities as they relate to the overall enhancement of program service delivery. |

Qualifications:
- Bachelor’s Degree with a major coursework in Sociology, Social Welfare, Psychology, Gerontology, Criminal Justice or other related social sciences.
- Two years’ experience as a Supports Coordinator; or have a combination of experience and education equaling at least six years of experience in public or private social work including at least 24 college-level credit hours in Sociology, Social Work, Psychology, Gerontology or other related social science.
- Must have knowledge of Pennsylvania’s intellectual disability and autism service system which includes successful completion of
- Person-Centered Thinking training
- Persons-Centered Planning training
- Three to five years experience in a supervisory position
- Five years working with the mental health/intellectual and developmental disabilities population
- Knowledge of the Intellectual and Developmental Disabilities system and commitment to Everyday Lives values
- Excellent people skills, proven ability to work in a team
- Attention to details and ability to work independently within established parameters
Physical Requirements/Working Conditions:
- Light work requiring little or no expenditure of physical effort. Travel and flexible schedule required with after hours, weekend and holidays on call time.
