Full Time Position (40 Hours a week)

Salary Range - $45,000 - $46,000

The Family Support Specialist (FSS) conducts need assessments and provides case management and tracking, based on established Camden Healthy Start (CHS) guidelines, policies and procedures for individuals in the preconception, prenatal, postpartum and interconception phases to ensure linkage to perinatal care and pediatric care for their infants. The FSS maintains ongoing relationships with community and health care providers, making referrals for program services.

Essential Functions

  • Maintains an active caseload of clients. * Provides assessment of all clients using approved program criteria, client intake and data collection/documentation tools. * Develops and maintains a service plan on all clients with client input regarding priorities. * Provides and documents case management services in compliance with program guidelines. * Identifies resources and link clients to required services. * Communicates with referring providers and other case managers involved with clients in accordance with program guidelines. * Ensure clients follow through with ongoing educational support and referrals for needed services. * Complete all required reports and other documentation in a timely manner. * Attends and participates in regularly scheduled supervisory meetings with Project Director, QI meetings with the Quality Improvement Manager, general Coalition meetings with the Project Director, training sessions, care coordination and case conferencing; attends other meetings as assigned. * Maintains client and staff confidentiality in compliance with operational policies and procedures. * Advocates on behalf of clients. * Conducts networking with community stakeholders and recruits’ community participation in collaborative problem solving around barriers to care and in linking clients to services. * Performs other duties as assigned.

Requirements

  • Bachelor’s degree in social work/human services or related field; Masters preferred.
  • Basic computer skills required.
  • 2 years’ skills and experience in case management and community outreach.
  • Experience with group process and parenting education helpful.
  • Knowledge of community resources.
  • Demonstrated leadership skills.
  • Good organizational skills; ability to prioritize work requirements.
  • Ability to work well with consumers and providers.
  • Bilingual English/Spanish required.
  • Experience with maternal and child health issues and target population a plus.
  • Valid driver’s license, car and current insurance policy.

Physical Requirements

  • Must be able to see to read documents and hear well enough to communicate with others.
  • Must be able to write, type on a computer, and use a telephone.
  • Must be able to operate a motor vehicle.
  • Must be able to lift up to 20 pounds.
  • Additional requirements to be determined with input from the Director and HR.

Work Environment

Travel regularly to client sites where traffic, weather and other environmental concerns may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions.
