Job Description:
Work with families and residents to provide resources and services throughout their stay at The Courvilles. This position requires a person who has a passion for “HumanKindness and Wellness for Life”. Must enjoy working with seniors.
- Manage and coordinate all the major aspects of the patient's care
- Communicate the relevant aspects of the rehabilitation and long-term care process to all necessary parties
- Assist in coordinating necessary and appropriate services
- Assuring financial reimbursement for services
- Promoting adequate documentation
Qualifications:
- Bachelor's degree In social work
- Min 5 years experience in rehabilitation and/or healthcare (Preferred)
- Excellent oral and written communication skills
- Strong organizational and interpersonal skills
- Negotiation skills
- Ability to work autonomously
