Reporting to the Clinical Director and responsible for coordinating and providing end-of-life care to terminally ill patients and their families. The RN Case Manager develops, implements, and updates individualized care plans in collaboration with the patient and family. The role involves delivering direct patient care, overseeing the interdisciplinary team, and educating patients and caregivers on disease processes, symptom management, and providing emotional support during difficult times. Care is delivered in the patient's home or any facility where the patient resides. Job Responsibilities : Presents a professional image and represents the Company in a positive, calm, confident, and competent mannerConducts in-home visits in a professional, courteous manner, projecting warmth during interactions with patient & family. Engages in purposeful conversation using appropriate body language and active listening skillsUtilizes critical thinking skills and nursing process to holistically assess patient and family, develop and implement an interdisciplinary hospice plan of care, initiate interventions and evaluate effectiveness.Educates family / caregiver(s) regarding plan of care, interventions, diagnosis and prognosis, medications, use, administration and side effects, how to provide care, use of equipment and supplies and how to contact Company for assistanceCompletes documentation of patient visit by verifying visit frequency and documenting accurate reflection of patient's condition during the patient visit; updates plan of care as needed.Concludes patient visit by providing encouragement to caregiver(s), ensuring emergency phone number (Call Center Number) and appropriate amount of supplies / medications are available.Qualifications RN with an active TN or Compact license BSN is highly preferredHospice certification is highly preferredMinimum of 2 years as practicing nurse Experience with Hospice, Oncology, Home Health, and / or long-term care is highly preferredExemplifies high level of written oral and listening communication skills, always respecting patient confidentialityFunctions consistently and collaboratively as an integral part of Patient Care Team and other teams (committees, etc.) bringing experience and education to contribute to optimal team functions and outcomes.Demonstrates a high level of work, personal and professional ethicsMust have a clear background check and driving record and a strong desire to work directly with residents in the healthcare field.
