Overview :
The Memory Care Coordinator is responsible for the integrity of the Neighborhood model. He/She is a coach, mentor and supervisor for any staff working in the Dogwood Neighborhood. The Memory Care Coordinator coaches, directs, and monitors the quality of care and the quality of life of the members in the neighborhood. The Memory Care Coordinator and is responsible for guiding the outcomes of the work, both individually and as a team for those working in the Dogwood neighborhood.
Job Description:
The role of the Memory Care Coordinator is to oversee compliance, competence, and operational issues and to provide monitoring and assistance to enhance the engagement of residents within the Dogwood. Interaction will all team members, residents and families will be in a manner that is respectful and collaborative. They will work in collaboration with managers/directors in other areas to insure engagement is being enhanced by all who work within the neighborhood.
Job Responsibilities:
- Act as a mentor and facilitate individual growth of all staff
- Train staff to engage members in a meaningful way
- Coach and mentor staff to support communication with families
- Emergency Preparedness
- Regulatory compliance
- Visit each resident daily to build relationships with the resident and their families
- Discuss observations and/or concerns about the care of the Members with the appropriate team member and/or appropriate manager/director
- Determine the education, information, training, coaching, and feedback needs of the team
- Collaborate with The Director of Nursing for any medical care and/or concerns and compliance
- Collaborate with the Certified Dietary Manager for the nutritional and hydration needs of the residents of the Dogwood Neighborhood
- Collaborate with the Environmental Services Director to insure the cleanliness and presentation of the neighborhood
- Continuously coach and nurture team’s interpersonal interactions
- Promote continuous growth and development of the neighborhood culture
- Review and coach the team’s decision making process and provide examples of effective approaches to decision making
- Plan and coordinate informational meetings with staff
- Plan and coordinate family functions as needed
- Attend team meetings to provide support and guidance
- Ensure and develop the team’s compliance with in-service requirements
- Collaborate with other managers and directors to enhance the relationship between the direct care team and the support teams
- Provide leadership, direction and oversight to other staff members coming into the neighborhood
- Ensure overall regulatory compliance
- Make employee life cycle recommendations to appropriate Director
- Review and submit budget to appropriate Director
Qualifications:
- Passion for working with elders
- Recreational Therapist, Social Worker, LPN or RN
- CPR/First Aid Certified
- Excellent communication skills
- Must have required Household Training including:
- Coaching Supervision
- Dementia Training
- ServeSafe
Education and/or Experience:
- Minimum of two years’ experience in working with adults in a group home setting.
- Minimum of two years’ experience working in a supervisory capacity in a hospital, skilled nursing facility, or other related medical facility strongly preferred.
- Must possess an understanding of the social and psychological needs of those confined to a long-term care setting.
- Knowledge of current DHEC regulations strongly preferred.
