Resident Care Manager/MDS Coordinator
Compensation:
Retention Bonus: $15,000
RN: $51.00 to $55.00 per hour (DOE)
LPN: $47.00 to $52.00 per hour (DOE)
Student loan forgiveness
Preference for RNs, but LPNs will be considered.
Job Summary: The Dalles Health and Rehabilitation Center is seeking a dedicated and compassionate Resident Care Manager/MDS Coordinator to oversee the care of 22 residents, including both skilled and long-term care populations.
Essential Functions:
Oversee daily care of assigned residents.
Manage, create, and maintain Care Plans in compliance with CMS regulations.
Conduct resident assessments and oversee MDS documentation to ensure regulatory compliance.
Track, report, and evaluate residents for Medicaid Complex Criteria.
Collaborate with interdisciplinary teams to optimize resident care and ensure accurate reporting.
Manage the MDS schedule, ensuring timely completion and successful submission.
Handle Certs and Recerts, ensuring compliance with CMS requirements.
Provide mentorship and support to nurses and caregivers.
Work closely with the Director of Nursing (DNS) to implement quality improvement initiatives.
Education, Licensure, and Experience:
Graduate of an accredited school of nursing (Required).
Current LPN or RN licensure in the state of employment (Required).
Demonstrated knowledge of nursing processes, particularly related to geriatric residents, long-term care, and rehabilitation.
