We are currently looking for a highly organized, detail-oriented, compassionate individual to fill a full-time Service Coordinator position in Clay and Platte counties. Candidates must be able to travel regularly to locations within the assigned counties and must be available to work Monday through Friday during typical business hours. Travel throughout the assigned counties to conduct regular business, including home visits, is typically 3-5 days per week.
Missouri First Steps is Missouri’s Early Intervention system (Part C of IDEA) for infants and toddlers, birth to age 3, with delayed development or diagnosed conditions associated with developmental disabilities. The goal of Missouri First Steps is to ensure that families have the necessary supports, services, and resources to raise healthy, happy, and successful children.
Under a contract with the Department of Elementary and Secondary Education (DESE), The Daulton Group, Inc. employs service coordinators in the Northwest Region of the state who are responsible for conducting all activities related to intake, eligibility determination, Individualized Family Service Plan (IFSP) meetings, Early Intervention Teaming (EIT) meetings, and transition meetings. Service coordinators must meet all state and federal timeline and process requirements.
Service Coordinators are responsible for conducting and completing all required processes, documentation, and scheduling and facilitating in-person meetings for the children on their caseload. The Service Coordinator will also be responsible for ensuring that all activities are conducted in accordance with the guidelines and requirements of the First Steps program.
This position is expected to be 40 hours per week.
Service Coordinators keep families in the forefront of all of their job activities and responsibilities, including, but not limited to:
- supporting families through the intake, eligibility, and IFSP processes by ensuring their understanding of the Missouri First Steps program's philosophy and processes;
- ensuring families are fully informed of their rights and understand the eligibility criteria and determination process;
- locating and understanding community resources;
- coordinating early intervention evaluations/assessments and services;
- collaborating with Early Intervention Team providers in order to ensure individualized, quality, consistent, and cohesive services are provided for each family; and
- building each family's capacity to best help and teach their child.
Service Coordinators employed with The Daulton Group, Inc. work from home offices with frequent travel throughout the assigned counties.
Position Requirements:
- Bachelor's Degree or Higher AND at least 3 years of documented experience working with families and young children;
- Valid Driver's License and auto insurance;
- Reliable transportation and the ability to travel within the assigned counties on a daily basis;
- Access to reliable and secure home internet
Preference will be given to candidates who:
- Reside in Clay or Platte Counties in Missouri
- Have experience with home visiting and working with families
- Have experience working with families of young children with disabilities
- Have experience with case management
