The Denver Hospice has an immediate opening for a field RN Case Manager on our Optio team.
LOCATION: Denver, CO
STATUS: Full-Time
SCHEDULE: Days, 8am - 5pm and on call shifts
PAY RANGE: $37.92 - $44.56 per hour
SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Paid Mileage/Shift Differentials/Stipends
MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile – one of the highest in the industry!

REWARDING WORK YOU WILL DO:
Provides assessment, supportive care and education to prospective patients and families regarding hospice services. Determines hospice appropriateness of the patient; assists in admitting patients to the hospice program. Acts as a liaison with assigned hospitals, facilities and physicians.
- Provides skilled nursing coordination of care to patients (neonates to geriatrics) as determined by the Plan of Care (POC). Uses education, mentoring, guidance and interventions to achieve goals of care.
- Collaborates across disciplines to coordinate patient care; participates in the POC in IDT/IDG meetings as assigned.
- Demonstrates appropriate nursing care, including communicating with attending provider for medications and supplies.
- Delivers accurate and timely documentation in Electronic Medical Record (EMR). Uses clear, concise and professional written and verbal communication.
- Uses sound nursing judgment and strong assessment skills to perform complex telephone interviews and triaging. Educates and assists patient/family with palliative treatments and Advanced Care Planning.
- Participates in community-related services, such as education, team training, staff/board committees and orientation.
- Supports patient and family with external resources such as home health, financial concerns, counseling, etc.
- Works with admissions staff to facilitate smooth transition from palliative services to hospice benefit and/or from hospice to palliative services.
- Promotes advance partnering with community health care agencies to expand the influence of palliative services.
Maintains effective working relationships with health care referral sources. Coordinates involvement of other palliative care service team members.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
- Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
- Minimum Experience: 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
- Required License: Current Colorado RN licensure. Valid Colorado State driver’s license and auto insurance.
- CPR certification required
PHYSICAL REQUIREMENTS:
- Ability to lift minimum of 35 lbs.
The Denver Hospice is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado’s Front Range and the Region’s Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip The Denver Hospice to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
