The Denver Hospice has an immediate opening on our South team for a Social Worker - MSW required (LSW or LCSW preferred).
LOCATION: Denver, CO
STATUS: Full-time
SCHEDULE: Monday-Friday, 8am-5pm
PAY RANGE: $61,422 - $74,880 annually
SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Paid Mileage/Shift Differentials/Stipends/Overtime
MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile – one of the highest in the industry!

REWARDING WORK YOU WILL DO:
Provides clinical psychosocial support to patient and family/caregiver in accordance with patient preferences, goals, and needs. The position works collaboratively with the Interdisciplinary Team/Group (IDT/IDG) as well as with facilities and agencies in the community.
- Works with Admissions Department to facilitate smooth transition from palliative services to hospice
services. - Completes initial and on-going psychosocial assessment of patient and family/caregiver, to include the
patient’s history, ethnic background, cultural & religious beliefs and the implications for the Plan of Care
(POC). - Establishes goals based on assessment and mutual agreement with patient and family. Serves as a contact
and/or advocate for patient/family and other providers. - Assists patient and family to prepare for, and cope with, disease progression, loss, aging and death.
Assesses and assists patients with advanced directives and care planning. - Evaluates patient and family psychosocial needs; provides consultative intervention with the individual
and/or family as part of the Plan of Care (POC). Provides appropriate referrals to community resources. - Conducts mental health screenings for patients using standardized mental health tools. Provides brief
psychotherapy interventions for depression and/or anxiety for patients. Collaborates with IDT/IDG and
referral source on mental health intervention and disposition needs to other mental health providers as
needed. - Develops a comprehensive discharge plan in collaboration with patient, family, staff and referral agencies
which identifies patient needs and preferences. Assists with discharge planning, financial matters and
community resources as appropriate.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
- Minimum Education: Master’s degree in social work from an accredited school of Social Work.
- Minimum Experience: One (1) year of social work experience in a health care setting.
- Required License: Current Colorado Social Worker license (LSW or LCSW) preferred. MSW must be willing to work towards licensure. Current CPR certification based on individual affiliate requirements. Valid Colorado state driver’s license and proof of auto insurance.
PHYSICAL REQUIREMENTS:
- Ability to lift/push/pull minimum of 30 lbs.
The Denver Hospice is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado’s Front Range and the Region’s Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip The Denver Hospice to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
