Program of Assertive Community Treatment (PACT)
$62,400 Annual Salary
Provides recovery-oriented treatment services to team Persons served and performs indirect
services as detailed below. Staff will be required to attend additional training on a range of topics such as evidence based practices, recovery-oriented treatment, trauma informed care, illness-management-recovery, and forensic specialties. Maintains a caseload of Persons served receiving outreach services.
Consumer Services
MA/MS and at least one year of experience in the field required. Interest in vocational rehabilitation population required. Experience with vocational- supportive counseling preferred. Valid driver’s license required. Skilled in the use of Microsoft Word.

Consumer Services
- Promotes consumer recovery and wellness in all actions.
- Establishes and maintains therapeutic, supportive relationships with all team Persons served.
- Assists Persons served in identifying their personal recovery goals
- Actively works with Persons served to develop recovery-oriented, person-centered individualized action plans (IAP), as assigned.
- Involves consumer’s Legally Authorized Representatives in planning
- Involves consumer’s family and significant others in planning where desired by consumer. Encourages consumer to involve family except when contra-indicated.
- Carries out IAPs consistently as written.
- Teaches and demonstrates good interpersonal skills with Persons served. Facilitates communications, gives direct and appropriate feedback, provides effective positive reinforcement, etc.
- Appropriately and effectively role models for Persons served through actions and appearance.
- Utilizes and teaches problem-solving techniques
- Assists Persons served in attending activities and appointments, providing transportation when appropriate. Teaches and assists Persons served with the maximum use of public transportation, as needed.
- Provides crisis intervention as needed, consistent with Crisis Prevention/Intervention plan and Center policies.
- Responds to emergencies appropriately.
- Provides back up for other PACT staff as necessary.
- Provides accurate observations/assessments of consumer behavior.
- Observes and documents consumer progress toward meeting objectives.
- Monitors and records consumer progress in all service areas.
- Respects Persons served’ rights, preferences and choices in all aspects of service delivery.
- Assume the lead role and participate in the provision of vocational services
- Develop clinical relationships with persons served which will support a potential vocation goal.
- Provide individual vocational-supportive counseling to enable Persons served to identify vocational strengths and problems, establish vocational or career goals and plans to reach them and recognize and target symptoms of mental illness that interfere with work.
- Plan and provide work related supportive services such as assistance with grooming and personal hygiene, securing appropriate clothing, wake-up calls and transportation.
- Teach job-seeking skills.
- Develop individualized jobs based on Persons served’ needs, abilities and interests.
- Conduct on the job performance assessments and evaluations, regular work review sessions with Persons served and their employers, on-the-job support and crisis-assistance contacts.
- Perform job coaching, problem solving, and support on and off the job site
- Coordinate with state vocational rehabilitation and other employments services.
- Receive training in ISP in order to facilitate appropriate use with Persons served.
