Job description

We are looking for a motivated The Psychiatric Mental Health Nurse Practitioner (PMHNP) who will provide outpatient psychiatric evaluation and medication management services at The Evolve Center. The Evolve Center is a Lawrenceville, GA based behavioral health agency specializing in helping individuals and couples navigate the challenges of trauma, emotional distress, and self-doubt. The PMHNP will provide psychiatric services to adult and youth clients from diverse backgrounds, including those struggling with anger, substance abuse, and chronic depression. The PMNHP will work out of one or more Evolve Center office sites in the Gwinnett County area. Services will be both office-based and via telehealth to serve clients in remote locations.
Responsibilities:
  • Responsible for providing psychiatric services to clients in a variety of clinical settings during scheduled hours. The scope of services includes outpatient psychiatric evaluations and medication management services.
  • Completes required training within prescribed timeframes.
  • Must possess and obtain a thorough knowledge of adult psychopathology, psychiatric nursing practices and medications.
  • Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of behavioral health disorders.
  • Prescribe, recommend, or discontinue drugs and treatments to treat mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement and the state’s Nurse Practice Act.
  • Knowledge of available community agencies and resources that support individuals with clients, while maintaining appropriate boundaries.
  • Collaborate with counselors, social workers, or other treatment team members to coordinate client care.
  • Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
  • Counsel clients during appointments, as necessary.
  • Design / participate in the development of individualized treatment plans, incorporating best practices into treatment interventions.
  • Examine or order laboratory or diagnostic tests for clients to provide information on general physical condition and mental disorders.
  • Collaborate with guardians, relatives, and significant others on client’s conditions and treatment when clinically appropriate.
  • Responsible for management and risk reduction in areas of responsibility and to report it if it is outside of areas of responsibility or seek assistance when needed to resolve the matter. Assures health and safety risks are controlled or removed, and a safe and therapeutic environment exists.
  • Maintains documentation as required, including medication administration record (MAR).
  • Prepare and submit assessments, progress notes, end of week and month reports, and summaries to entities/agencies (funders, insurance companies, etc.) as required.
  • Manage, monitor, and inform leadership as it relates to productivity and making sure to maintain 91% productivity for each service we provide.
  • Return emails and calls promptly (within 24 hours of receipt or sooner if clinically necessary)
  • Working knowledge of crisis intervention principles, tools, and techniques.
  • Ability to assess crisis situations for safety and security and respond to client emergencies and be accessible to The Evolve Center 24/7 crisis clinicians.
  • Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions.
  • Responsible for maintaining customer relations with all agency staff and clients.
  • Maintain medical records and submit at least 10% of records to the supervising Psychiatrist for bi-weekly review in accordance with the Collaborative Practice Agreement for utilization review.
  • Records all services performed for accurate billing for services provided in compliance with all local, state and federal laws, as well as payor rules and regulations. Submits billing documents in a timely fashion.
  • Looks for ways to improve and promote quality. Provides teaching and education to clinical staff, if requested.
  • Follow all procedures and protocols as per The Evolve Center policy, federal and state regulations, standard practice, and in accordance with the Collaborative Practice Agreement.
  • Actively participate in all agency and department education, training, and in-services to confirm understanding of new information or changes to established clinical policies and procedures.
Requirements:
  • Must be certified by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric mental Health Nurse Practitioner (PMHNP).
  • An unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Georgia.
  • Must have a valid DEA number and unrestricted DEA license in Georgia.
  • A state-issued Controlled Substance Registration (CSR) in Georgia.
  • Registration with the state Prescription Monitoring Program (PMP) in Gwinnett County and for Georgia, where direct services will be provided.
  • No Suspension/exclusion /debarment from participation in federal health care programs (e.g. Medicare, Medicaid, CHIP).
  • Maintain credentials and licensure with all major insurance companies and licensing boards with the assistance of Evolve Center of Counseling credentialing staff.
Other Skills Required:
  • Experience in delivering high-quality, client-centered care to individuals and couples.
  • Having a working knowledge of basic to intermediate electronic health records (EHR) systems is preferred.
  • Ability to work as a member of an interdisciplinary team, as well as members of the community.
  • Strong social, communication and written skills. Must have the ability to develop rapport and be perceived by clients and staff as approachable. Ability to multi-task; while maintaining accuracy and organization.
  • Highly proficient use of Outlook (email, calendar, contacts, tasks).
  • Highly proficient use of Microsoft Office Suite (Word, Excel, PowerPoint).
  • Experience with CRM software.
  • Able to multitask, prioritize, and manage time efficiently.
  • Ability to work effectively on multidisciplinary team.
  • Ability to analyze and interpret general and professional documents. Ability to write routine reports and correspondence. Ability to effectively present information in one on one and small group situations.
  • Must possess the ability to make independent and informed decisions when circumstances warrant such action.
  • Ability to foster teamwork; train and direct the work of team members and colleagues.
  • Excellent organizational skills, including the ability to work productively and make independent decisions.
