I. Qualifications a. Bachelor’s Degree in Social Services b. One (1) year experience working with clients c. Ability to communicate verbally and in writing d. Desirable to have an understanding of strength-based, family and solution focused treatment strategies for working with at risk client.
II. Organizational Relationships and General Description a. Works under the direct supervision of the Program Manager. b. Works closely with the Life Skills Advocates/Sup. And Program Manager,
Interdisciplinary Treatment Team and all other staff members.
III. Description of Key Responsibilities a. Case Management i. Prepares and maintain a permanent and organized filing system of clients’ case records per DHYS, JCAHO, DJJ, LOC, MAAC, ORS, and
STAC regulations within 24 hours of admission to the program. Files are regularly updates with required information. ii. Files progress notes and other documents within 10 days of receipt unless documents have been revised and/or removed from files, and then within 24 hours. iii. Communicates with Client Placement Specialists, case managers, parents/guardians, and family members effectively as seen in complete and timely documentation of correspondence log, 30 days treatment plan reviews, 90 days treatment plan reviews, case panel reviews, LOC, DJJ,
DFCS, MAAC, and STAC utilization reviews. iv. Ensures that the admission packet is completed at time of admission along with the Program Manager. v. Ensures that Discharge Statements are completed and signed at time of discharge. vi. Ensures that Discharges Summaries are completed and signed within fourteen days discharge. vii. Manages clients’ personal funds from allowances per policy and procedure. viii. Ensures that clients’ clothing needs are met and billing followed per policy and procedure. ix. Attends and actively participates in LOC, DJJ, MAAC, and STAC utilization reviews, Case Panel Reviews, 30 days treatment plan reviews,
90 days treatment plan reviews. x. Completes the Client Adolescent Functional Assessment Scale and
MATCH Profile Instrument, Part II on funded clients upon admission, quarterly thereafter and at discharge. xi. Ensures that the Client Satisfaction Questionnaire is completed by clients upon discharge. xii. Ensures that the Parent/Legal Guardian/Placement Program Satisfaction
Survey is completed upon discharge.
The Family Life Center b. Assist with groups – Weekly Therapy Groups and Multi-Family Groups as scheduled. i. Follows treatment plan as outlined. ii. Completes documentation/charting per DHYS, LOC, JCAHO, MAAC,
ORS, STAC regulations. iii. Participates with Individual Family, Multi-Family Groups and family visitation, along with Clinician, as scheduled by Program Manager. iv. Actively plans discharge alternatives with clients, families, case managers, probation officers and client placement specialists from admission through actual discharge. Becomes familiar with private and public services in Georgia and other states as required per referral. v. Begins discharge planning at time of admission; discusses plans with clients, parents/legal guardians, families, custody holders/caseworkers and Client Placement Specialists; utilizes treatment plans to monitor discharge plans. c. Participates in on-call schedule i. Responds within ten (10) minutes and documents every call immediately. ii. Provide assistance as needed. iii. Completes documentation/charting per DHYS policies and procedures. d. Facilitates Interdisciplinary Team meetings i. Attends and actively participates in the treatment planning process. ii. Facilitates interdisciplinary team meetings effectively to review all caseloads/units. iii. Completes Initial Treatment Plan within three (3) days of admission along with client, parents/legal guardians, family members, clinical managers, clinicians and Interdisciplinary Team members. iv. Revises monthly treatment plans upon receipt from Clinician. v. Completes Master Treatment Plan within thirty (30) days of admission, along with client, parents/legal guardians, family members, case managers, and clinician and Interdisciplinary Team members. vi. Encourages and empower clients, parents/legal guardians and family members to take an active role in the treatment planning process. e. Staffing or Treatment Plan Reviews i. Facilitates meeting with Case Managers/Probation Officers,
Parents/Legal Guardians, Family Members and Clients to review client status. o Facilitates 30 day treatment plan review of clients upon admission. o Facilitates first 90 day treatment plan review and then in 90-day increments thereafter until discharge. f. Supports the philosophy that promotes strength-based, community-based solution focused and family focused activities. i. Models, supports and implements the philosophy in development of treatment plans and various activities/events. g. Assists with the coordination of home evaluations/assessments, therapeutic home visits and other wraparound services for clients along with the Clinician.
The Family Life Center i. Attends and actively participates in scheduled home evaluations/assessments, therapeutic home visits and other wraparound services offered for clients. ii. Works closely with Clinician, clients, parents/legal guardians, families, child care staff, case managers and others to obtain needed services in respective communities prior to discharge. iii. Assists with the arrangement of transportation, meals and medications, if needed, as requested by Clinician. iv. Assists with the preparation of needed documentation prior to home visits and reviews documentation upon return and addresses any concerns as requested by Clinician. v. Encourages and empower parents/legal guardians and family members to take an active role in the treatment planning process. vi. Maintain confidentiality of case records and have the ability and authorization to release information about clients in accordance with program release of information policy section 5.03.
