Job Summary:
Family Resource Network is hiring a Community Health Navigator, reporting to the Director of Public Health Programs, to support the following programs: Information & Referral, Health Insurance Navigator, National Caregivers Conference, and statewide Caregiver Coalitions. This hybrid position requires in-office presence in our East Windsor corporate office at least two times per month and travel throughout New Jersey as needed for outreach and events.
Job Description:

Plan, execute and evaluate annual National Caregivers Conference including but not limited to sponsorship outreach, event registration, and speaker selection

Oversee management of county Caregiver Coalitions including attending monthly meetings, managing social media, publishing quarterly newsletters, and building strategic partnerships

Connect consumers to information and referral services related to disability, nutrition, health insurance and caregiving needs

Assist consumers in navigating complex systems such as healthcare access and housing support through individual and community outreach, education, and enrollment

Complete all required state and federal Navigator training and facilitate enrollment in qualified health plans through GetCoveredNJ and NJFamilyCare

Maintain accurate records and documentation of client interactions and referrals through internal recording database

Conduct community outreach to connect with individuals who may be underserved or unaware of available resources

Position Requirements:

Bachelor’s degree in social work, psychology, healthcare administration, public health, nursing or related field required. Master's degree in public health or related field preferred.

Experience in social services, healthcare, case management, or caregiving.

Experience planning, executing and evaluating large-scale programs or conferences.

Familiarity with community resources, elder care, disability resources, or mental health support.
Knowledge and Skills:

Strong understanding of social determinants of health and barriers to accessing services.

Knowledge of health insurance and disability benefits programs including Medicaid, Medicare, and the Affordable Care Act.

Ability to navigate healthcare, legal and social service systems related to intellectual and developmental disability, elder care, aging and caregiving.
Additional Qualifications:

Certification as Family Support Specialist, Certified Senior Advisor, Community Health Worker, or similar credential desirable.

Experience working with individuals with intellectual or developmental disabilities and their families/caregivers.
