Wellness Director
Position Summary: The primary purpose of the Wellness Director is to coordinate the development and implement each tenant’s plan of care. This is based upon each tenant’s individual physician orders, emotional, social, nursing, and personal needs and goals. The Wellness Director manages the nursing operations, directs completion of documentation and Service Plans, ensures delivery of services expressed in individual Service Plans in accordance with the facility's established policies and procedures, applicable laws and regulations and the directions of your supervisors, who include the Director of Clinical Services and Operations, Executive Director, members of the Regional management team, and other members of Arcadia’s Corporate leadership to whom such persons report, in order to assure that the highest level of support for the facility's residents is maintained at all times. As a Wellness Director, you are delegated the authority, responsibility, and accountability necessary for performing your assigned duties.
Essential Duties:
Serve as House Supervisor when in facility.
Serve a Clinical Lead and provide on-call duties, as required.
Daily assessment of tenant’s experiencing change of condition.
Oversee medication management program.
Planning tenant services.
Complete assessments as required.
Create Service Plans based on assessments.
Oversee nursing employees, assigning and directing nursing personnel.
Manage staff education, by planning in-services to improve knowledge and skills.
Manage tenant education.
Direct incident investigations.
Direct abuse investigations.
Manage grievances/complaints.
Review documentation and perform screening of potential tenants to determine appropriateness for placement. Collaborate with Director of Clinical Services and Operations as needed.
Maintain accuracy and completion of tenant charts.
Ensure tenant and employee free of communicable disease.
Supervise and lead the Memory Care Team (if applicable)
Serve as Committee Chairperson for Quality Improvement Committee, Tenant Satisfaction Committee, and Infection Control Committee
Assure that established infection control and standard precaution practices are supported.
Use protective clothing/devices when performing tasks that involve the handling of infectious garments and linen and/or blood/body fluids.
Follow established safety precautions when performing tasks and using equipment or supplies.
Report all equipment malfunctions and breakdowns to the supervisor as soon as possible and keep the supervisor informed of supply needs and equipment needing replacement.
Assure nursing schedules are followed.
Report all hazardous conditions or equipment to the supervisor.
Report all accidents/incidents to the supervisor.
Maintain the confidentiality of resident information and honor his/her personal and property rights.
Attend department meetings as directed.
Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed.
Turn in Days Off and payroll special request forms, supply sheets, and annual medical examinations in a timely fashion.
Follow established fire, disaster, and evacuation policies and procedures.
Perform other related duties as assigned by the Executive Director and/or Director of Clinical Services and Operations.

Qualifications:
Must have, as a minimum, a Licensed Practical Nurse or Registered Nurse license, unencumbered within the State of Illinois.
Must be BLS/CPR Certified
Must have had previous Supervisory experience.
Must have strong assessment skills.
Must demonstrate the ability to make independent decisions when circumstances warrant such action.
Must demonstrate the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must support the care and use of supplies, equipment, the appearance of work areas, and perform regular inspections of the nursing department and equipment for sanitation, order, safety, and proper performance of assigned duties.
Ability to communicate efficiently with physicians, pharmacists, and family members.
Must complete CARES Alzheimer’s Certification and EssentiALZ Certification for Memory Care units.
Physical Demands:
Must be able to move intermittently throughout the workday.
Must be able to cope with the mental and emotional stress of the position.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
