Schedule: Part-Time, 20 Hours Per Week
The Gardens at Orangeville is seeking a skilled and compassionate MDS Assessment Coordinator (RN or LPN) to join our team. In this vital role, you will oversee resident assessments, ensure compliance with state regulations, and develop care plans that reflect the highest quality of care for our residents.
Key Responsibilities
- Conduct routine and as-needed resident assessments, including those triggered by condition changes, hospital returns, or regulatory requirements.
- Ensure accurate observation, assessment, and documentation of residents’ health conditions and communicate findings effectively.
- Coordinate and oversee the timely completion of assessments and Care Area Assessments (CAAs) by interdisciplinary teams (e.g., Social Services, Activities, Dietary).
- Develop and maintain individualized care plans based on Resident Assessment Protocols (RAPs).
- Prepare and manage MDS schedules for the interdisciplinary team.
- Accurately transmit MDS data to the state as required, maintaining detailed transmittal logs and records.
Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in the state of Pennsylvania.
- Minimum of one (1) year of experience in Long-Term Care as an MDS Assessment Coordinator.
- Must be 18 years or older.
