Case Manager-Transitional Living
Summary Statement:
Case Manager will perform case management duties according to established regulations, policies, and procedures. Case Management Specialist is responsible to manage a multiple caseload of transitional living clients and be an advocate for client needs.
Duties and Responsibilities
1. Determine level of care based on placement guidelines and the ASAM
2. Determine appropriate funding arrangements and recommend services for clients
3. Orients clients to the program, policy, procedures, and treatment process of The GateHouse
4. Develop and implements a comprehensive service plan based on the results of the assessment process
5. Updates the clients service plan as needed
6. Coordinates and provides access to available to drug and alcohol ancillary services including housing, childcare, medical, and financial services
7. Conducts client follow-up at 30, 60, and 90 days after discharge of Outpatient program
8. Monitors client treatment progress and performs utilization review activity
9. Prepare written reports and maintains client records
10. Ensures continuity of client care through discharge planning and coordination with community services
11. Adhere to DDAP Treatment Manual to include entry of applicable date into PA WITS
12. Assist and direct clients in meeting self-directed recovery goals
13. Perform other related duties as assigned
Qualifications
1. Bachelor’s Degree in Chemical Dependency, Sociology, Social Welfare, Psychology or a related field; and one year of direct service experience performing counseling or case management work in drug or alcohol treatment settings or A Bachelor’s Degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences; and one one-year direct service experience performing, counseling or case management work in alcohol or other drug treatment settings
2. A valid driver’s licenses and must be an approved driver according to GateHouse’s MVR request
3. Training or education in the areas of Case Management, ASAM, Screening and Assessment, and Treatment planning preferred
4. Must be willing to obtain CAAP or other related certification/licenses within 2 years of employment
Knowledge, Skills and Abilities
1. Knowledge of social case work principals and methods
2. Knowledge of addiction as a disease and its treatment
3. Knowledge of current social, economic, and health problems
4. Ability to establish and maintain an effective working relationship with clients from various cultures and social economic backgrounds
5. Ability to communicate effectively, both orally and in writing
6. Ability to interpret regulations, policy, and procedures and apply them accordingly
7. Ability work with persons with physical, mental or emotional disability’s or who are economically disadvantaged or who are involved with the criminal justice system
8. Ability to establish and maintain effective working relationship with other members of staff, outside agencies, and institutions and the general public
Physical Requirements and Work Environment
1. Must be able to regularly life and or move objects 25-50lbs
2. Must be willing to travel throughout Lancaster to meet clients
3. Must be willing to travel short and long distances to accommodate the needs of the client
4. Frequently required to stand, walk, stoop, knee, crouch, or crawl
5. Occasionally required to sit, climb, or balance
