Position Overview:
The general purpose of the Housing Stabilizer Case Manager role is to provide 1:1 housing stabilization support for members who have exited TGP’s Elati Village Micro-Community, a non-congregate shelter specifically serving women individuals. Using the Critical Time Intervention approach, the Housing Stabilizer Case Manager will play an essential role in helping members access necessary support and resources to maintain housing in collaboration with HOST’s Housing Central Command team. This position will require a high level of coordination across multiple teams and programs as well as the ability to travel and transport members using a personal vehicle throughout the city.

Core Accountabilities
- Demonstrate respect, cultural humility, and knowledge of trauma informed care in interactions with TGP members, staff, and collaborative partners.
- Works directly with program participants in the community doing in-home visits, providing trauma-informed and member-directed housing stabilization support using culturally responsive and evidenced-based practices.
- Evaluates progress of program participants and adapts case management plan and/or interventions as needed to increase engagement.
- Provides crisis intervention as needed.
- Collaborates and communicates with the Elati Village team, TGP programs, and external partners to ensure high quality care.
- Supports members in accessing services that aid in recovery from homelessness, substance misuse, mental health issues, and trauma, with the goal of achieving greater stability.
- Receive training for TGP’s member database, ETO, and the Homeless Management Information System (HMIS) and maintain monthly compliance with ETO and HMIS data tracking, assessments, etc. Enroll members into HMIS and ETO and follow data standards including case notes and member documentation.
- Complete training and utilize a Critical Time Intervention approach to case management as described in the HOST Manual.
- Stabilizers will receive and support members referred through Housing Central Command.
- Stabilizers will conduct a Housing Stability Plan assessment with members prior to placement in their permanent housing unit.
- Attend bi-weekly case conference with other Housing Central Command housing providers.
- Submit complex member cases to the Denver complex case review.
- Transport members in personal vehicle as necessary.
- Meet with supervisor once a week to discuss Critical Time Intervention stabilization for each household.
- Collaborate and communicate effectively with landlord partners and Housing Connector.

- Must have skills in crisis management, conflict management, and de-escalation.
- Previous housing navigation experience a plus
- Lived experience of homelessness, housing instability, or poverty a plus
- Strong understanding of trauma-informed care principles and skills
- Ability to listen compassionately and suspend judgment in order to accommodate the needs of individuals and include them in a highly diverse setting
- Ability to multi-task in a unique, fast-paced environment
- Ability to work independently or with a team
- Critical thinking and problem-solving skills
- Current knowledge of, or desire to learn, how to operate a computer and other office machinery, such as a copy machine or printer, and office software, such as Microsoft Outlook, Word, Excel, and other programs.
- Must have the ability to transport members throughout metro Denver using a personal vehicle. Mileage reimbursement provided.
