Position Overview:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN at our practice, you will be responsible for providing high-quality nursing care to our patients while maintaining the highest standards of professionalism and patient satisfaction.
\Skills, Knowledge and Abilities
- Conduct comprehensive patient assessments and develop collaborative treatment plans.
- Promote patient empowerment and self-determination; assist with adherence to treatment plans and self-management goals.
- Monitor, evaluate, and document patient progress.
- Collaborate with outside service providers to coordinate patient treatment and manage referrals.
- Record all interventions in patient charts and produce service delivery and outcome reports.
- Communicate through personal contact, phone, email, and regular mail; return messages promptly.
- Work with Chief Operations Officer and Clinical Director on data collection for outcome measurement reporting.
- Report patient progress in files and to medical staff.
- Collect and prepare laboratory specimens; coordinate with lab companies as needed.
- Perform basic laboratory and point-of-care testing.

What Expertise You Will Bring
- Oklahoma Licensed Professional Nurse (LPN) License. Must have a current and valid LPN license in the state of Oklahoma.
- Effective Communication. Strong ability to communicate with diverse individuals, including patients, families, and healthcare professionals.

Physical Requirements: While performing the duties of this job, employees are regularly required to sit, walk and stand; talk and hear, both in person and by telephone. Requires sitting, standing and walking for extensive periods of time. Must have strength sufficient to assist with lifting and transferring a patient and perform CPR. Must have sufficient mobility to sit, stand, bend, stoop, and bend down to the floor; ability to move around rapidly; and to move in small, confined areas.
