The Palms of Largo Senior Living Community is seeking a full-time Clinical Liaison to join their community.
This position requires the individual to hold either an LPN or RN (preferred) license, with a minimum of 2 years of experience working in an assisted living community.
This individual also needs to have prior leadership experience and understands the principles and practice of teaching & communication while using their creativity, integrity, and initiative to assist both residents and our communities simultaneously.
The Clinical Liaison plays an integral role in the success of our team, our community and creating a place our residents are proud to call home. The primary purpose of this position is to utilize a resident centered approach to caring for older adults or others facing ongoing health challenges. This model assists residents and their families in attaining the resident’s maximum functional ability while encouraging safe independence throughout the continuum of care within the local senior living community and the medical professional community.
The Palms of Largo is composed of 6 facilities, which offers a range of care options made possible in part by its unique licensing which affords residents seamless transfers within the communities on our campus should the need for a higher-level care or services arise. As a result, our residents remain connected to their respective neighborhoods, friends, familiar surroundings, and the services and team members they have come to know.

- Determine appropriateness of resident admission and level of service needed in collaboration with facility Director of Nursing (DON).
- Communicate with other departments to ensure continuity of the resident’s total regimen of care.
- Perform monitoring of residents' health status through periodic direct observation in hospital and rehabilitation. Make daily rounds on residents of concern.
- Assessment/evaluation and monitoring of staff.
- In-service hospital systems and rehabilitation facilities on The Goodman Group policy and product lines.
- Advise new residents/families members on the facility.
- Make rounds with health care providers in the hospital/rehab/ALF setting.
- Complete initial assessment/evaluations and develop interim plan of care and deliver to DON.
- Communicate with healthcare provider to review treatment plans, complete orders, progress notes, safe discharge, etc. in accordance with established policies.
- Review resident’s medical record for specific treatment plans, medication orders, diets, labs, etc., for implementation and follow up.
- Education, planning and problem solving with the resident and with the primary care giver/POA/family.
- Update, review and communicate with staff the plan of care that identifies the goals, problems, approaches, and revisions based on nursing needs, psychosocial and new healthcare provider orders.
- Assistance with clinical programs as directed, (e.g. dementia, wound care, quality assurance, infection control, admissions).
- Meet and resolve issues with residents and /or family members; report significant issues to the Director of Nursing or Executive Director.
- Utilize internal systems for tracking referrals, inquiries, tours, follow up contacts, admissions, discharges, average daily census and occupancy, etc.
- Make one to one marketing contacts with targeted sources and strategize to educate on services and request referrals.
- Serve as a liaison between the Administrator, medical staff and other professional and supervisory staff and community referral sources.
- Coordinate special functions with hospital systems.
- Coordinate with marketing team to schedule tours of the facility with targeted referral sources.
- Identify referral sources that may require in-service education provided by the community.
- Direct Sales calls to potential referral sources with the goal to receive a referral and ultimately admission to the community.
- Review and strategize marketing on a weekly to assure maximum marketing potential to include comprehensive initial and follow up contact with target audience.
- Identify new potential referral sources and strategize to educate within the hospital systems, Long Term Care facilities and rehabs.

- One (1) year of experience in healthcare required 2 years’ experience in homecare or assisted living preferred.
