Social Worker
The Grand Pavilion for Rehabilitation and Nursing at Rockville Center is looking for a talented and hard-working Social Worker to join their team.
The Grand Pavilion for Rehabilitation and Nursing at Rockville Center is a facility with 158 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 60 short term residents as well as a long term population. The Grand Pavilion for Rehabilitation and Nursing at Rockville Center team, with backgrounds in acute, sub-acute, long term and outpatient settings would provide an excellent setting for learning and growth.
Social Worker Qualifications:
- Must have a bachelors in Social Work or related field- LTC MDS experience preferred.
- Must have the ability to work independently and multitask.
- Must have a positive attitude and be able to provide excellent customer service.
- Must be familiar with the Electronic Medical Records programs ; preferably PCC
Social Worker Functions:
- Meet with patient/residents and their family’s to gather information and design a safe, secure discharge plan for each resident..
- Complete all necessary forms/documents for each patient in a timely fashion as per facility dictates.
- Work with other departments to assist patients/residents in any way possible.
- Make community referrals for each discharged patient, as indicated.
