Position: Licensed Practical Nurse (LPN)
The Grand Rehabilitation and Nursing Center at Pawling, located in Pawling, NY, is actively seeking LPNs for all shifts. We offer full-time, part-time, and per diem positions to suit your schedule.
LPN Responsibilities:
- Implement and coordinate care delivery in collaboration with physicians and care associates.
- Conduct regular health and wellness checks for residents.
- Assist in developing and updating individualized service plans for residents.
- Maintain open communication with residents, families, and physicians regarding their care needs.
- Utilize the nursing process (assessment, planning, implementation, evaluation) to ensure optimal resident care.
LPN Requirements:
- Current and valid NYS LPN License in good standing.
- Ability to work effectively in a team-oriented environment.
- Prior experience in a nursing home or hospital is preferred but not required.
