POSITION SUMMARY:
This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening
community through youth development, healthy living and social responsibility. The Community Care Worker will
serve as a member of the Y Wellbeing Initiative team. The Y Wellbeing Initiative offers members and the
community a myriad of opportunities to enhance and support their mental and emotional health and wellness. In
accordance with the mission of the Y Wellbeing Initiative, the Y is happy to offer the position of Community Care
Worker (CCW). The Community Care Worker is a key role within the Greater Scranton YMCA’s Moses Taylor
Foundation Grant Project, which is aimed at improving access to mental health services and reducing barriers to
care in Northeastern Pennsylvania. This position will provide direct support to individuals in community settings,
facilitate evidence-based mental health programming, and coordinate referrals to wraparound services (e.g.,
Medicaid, housing, food assistance). The CCW will work collaboratively with healthcare providers, community
partners, and YMCA staff to ensure seamless transitions to care and promote long-term mental well-being.

ESSENTIAL FUNCTIONS:
1. Oversees Administrative Aspects of Programming to Ensure Goals & Objectives are Met
2. Works in Collaboration with Program Director
3. Facilitates the COPE Program (evidence-based)
4. Facilitates the R1 Learning Program (evidence-based)
5. Delivers Direct Individual Support (i.e. individual support with, and through, mental health screenings,
assessments, personalized care plans and other case management related services).
6. Attends Various Community Events / Spreads Program Awareness
7. Provides Support relating to Mental Health Advocacy and Education
8. Facilitates Self-Regulation Workshops for Various Populations
9. Responsible for Program Reporting and Data Analysis
10. Suggests and Implements Program Improvements If/When Necessary
11. Conducts Community Outreach & Program Partner Development
12. Problem Solving and Strategy Sessions as Needed
13. Performs Other Duties as Assigned.

QUALIFICATIONS:
Bachelors degree in related field or equivalent experience. Master’s degree preferred.
Five years related experience (e.g., social work, counseling, and/or youth development) preferred.
Minimum age of 21.
Within 30 days of hire, completion of Child Abuse Prevention, Working with Program Volunteers, CPR, First
Aid, AED, and Bloodborne Pathogens training.
Fulfillment of state-specific hiring standards and completion of YMCA program-specific training.
Ability to relate effectively to diverse groups of people from all social and economic segments of the
community.

Initiative Team by staff at all levels throughout the Greater Scranton YMCA.
3. The Program Director is confident that the Community Care Worker (CCW) is performing at a high
level of productivity and professionalism.
