Occupational Therapist (OT) - Per Diem or 1099 Contractor
The Grove Health & Rehabilitation Center is seeking a passionate and dedicated Occupational Therapist (OT) to join our team on a per diem basis within our in-house subacute rehabilitation unit. In this rewarding role, you will work closely with residents to help them regain independence and improve their quality of life through personalized occupational therapy interventions.
Key Responsibilities :
- Patient Assessment : Conduct comprehensive evaluations to assess residents’ physical, cognitive, and emotional needs related to daily living activities.
- Individualized Treatment Plans : Develop and implement tailored therapy plans that address each resident's specific rehabilitation goals.
- Therapeutic Interventions : Utilize a variety of therapeutic techniques and activities to enhance residents’ functional abilities and self-care skills.
- Patient and Family Education : Educate residents and their families about therapy processes, adaptive techniques, and home exercise programs to support recovery.
- Collaboration : Work collaboratively with a multidisciplinary team, including physical therapists, speech-language pathologists, nurses, and physicians, to provide comprehensive patient care.
- Progress Monitoring : Regularly assess and document patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
- Documentation : Maintain accurate and timely documentation of evaluations, treatment plans, and progress notes in accordance with regulatory standards.
