The Grove Health & Rehabilitation Center is seeking a dedicated, compassionate, and skilled Physical Therapist, PT to join our team on a full-time basis. In this role, you will provide high-quality physical therapy services to patients in a subacute rehabilitation setting, focusing on improving mobility, functionality, and overall quality of life.
Key Responsibilities :
- Conduct comprehensive evaluations of patients’ physical abilities and needs, developing personalized treatment plans.
- Administer therapeutic interventions to address mobility, strength, and functional limitations.
- Collaborate with multidisciplinary team members, including nurses, occupational therapists, and physicians, to ensure optimal patient care.
- Educate patients and their families on exercises, techniques, and lifestyle changes to improve rehabilitation outcomes.
- Monitor patient progress, adjusting treatment plans as necessary to ensure continuous improvement.
- Document all patient evaluations, treatment sessions, progress, and outcomes in compliance with facility policies and regulatory requirements.
- Maintain professional development by participating in continuing education opportunities and staying up to date with the latest therapy techniques and practices.
- Adhere to infection control procedures, safety protocols, and organizational policies to ensure a safe and effective therapy environment.
