We are seeking a dedicated and compassionate Resident Care Coordinator to join our team! This position requires a Licensed Vocation Nurse (LVN) who is passionate about providing excellent care and support to members and their families.

This particular role is looking to hire someone that can review resident records, follow Watermark systems, and ensure positive outcomes for residents and the community. - Review 24-hour log
- Receipt and record review for new move-in
- EMR Dashboard review and update
- Census review and update
- Review of incidents and regulatory required reporting
- Pharmacy Order updates in eMAR
- Document uploading
- Complete resident assessments and service plans
- Attend and participate in meetings
- Able to work weekends and holidays
- Able to work in a fast-paced environment
