Position Summary
Hand2Paw is seeking a Part-Time Case Manager to provide socio-emotional support and connect program participants to essential community resources. The ideal candidate will work closely with youth to identify barriers affecting their program participation, stability, and employability while making appropriate referrals to social services. This role is ideal for someone looking for a flexible, part-time opportunity to make a meaningful impact on young adults facing adversity.
Key Responsibilities
- Conduct brief needs assessments with program participants to identify key challenges affecting their well-being and program success.
- Provide short-term, solution-focused support through individual check-ins and referrals to external services.
- Connect youth to community resources such as housing support, mental health services, transportation assistance, and employment programs.
- Maintain limited but essential documentation of participant interactions and referrals while respecting confidentiality. This includes but is not limited to maintaining data records of attempted and successful referrals made.
- Collaborate with program staff to identify youth who may need additional support and provide guidance on best practices for working with vulnerable populations.
- Offer guidance and crisis support as needed, ensuring youth have a trusted resource for immediate concerns.
Qualifications
- Master’s degree in Social Work (MSW) or a related field preferred; Bachelor’s degree with relevant experience will be considered.
- At least 2 years of experience in social work, case management, or youth services, preferably working with vulnerable or system-involved youth and young adults.
- Knowledge of community resources in Philadelphia, including but not limited to housing, mental health, and workforce development services.
- Familiarity with animal-based interventions and an understanding of the therapeutic benefits of human-animal interactions preferred.
- Strong interpersonal skills with the ability to engage and build trust with youth.
- Ability to work independently with minimal supervision in a small team environment.
- Familiarity with trauma-informed care and a strengths-based approach.
Work Schedule & Compensation
- Part-time position: 8-12 hours per week for an estimated 6 months
- Compensation: $30/hour
