POSITION SUMMARY
This position is responsible for coordinating and providing transitional care services for program clients up to and after the completion of the residential program. Services include support in accessing vocational, housing, family and educational resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential functions include but are not limited to the following:
- Collect, maintain and report relevant data pertaining to client completion of the program
- Develop case plans with Phase clients and oversee progress
- Develop relationships with service and community partners to meet client needs
- Assist in connecting clients to community resources to provide housing, education, family support and vocational opportunity
- Facilitate Silver Chip Support Group
- Facilitate Peer Mentor Clinical Training
- Manage Life Skills Class for program clients, including recruiting and scheduling presenters
- Oversee GED program for all program clients including biweekly meetings with participants, scheduling tests/classes and maintaining accurate data spreadsheets of client participation
- Serve as liaison for clients involved in legal, justice or family court systems
- Provide documentation of client participation and completion
- Provide client support with children and family issues

QUALIFICATIONS / REQUIREMENTS
- High School Diploma or Equivalency required/ Bachelor's Degree preferred
- Some experience working in residential recovery program or as a case manager preferred
- Working knowledge in Microsoft Office (especially Outlook, Excel and Word)
- Must be able to spend extended periods of time on your feet (walking, standing)
- Ability to project a professional image
- Must be able to lift 50lbs. on occasion
- Valid Driver's License required
