POSITION SUMMARY
The Veteran’s Case Manager is responsible for assessing the needs of the veterans, designing a case management plan, and coordinating all activities related to services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential functions include but are not limited to the following:
- Assessed veterans for service needs
- Develops and monitors an individualized case management plan based on the needs of the client
- Maintains proper documentation in client files
- Maintains an up-to-date roster of current clients
- Attends staff meetings as scheduled
- Works with VA liaison on veteran service eligibility, housing, and goal progress
- Works with Coalition for the Homeless to enroll clients in housing programs and/or monitor progress in housing
- Conducts twice weekly and random room checks to ensure cleanliness and proper condition for move in/out.
- Connects veterans with community and employment resources
- Develops and maintains relationships with veteran community providers
- Serves as liaison for clients involved in other systems such as legal and criminal justice
- Coordinates with other THP departments as needed
- Participates in all VA case conferences and technical calls
- Teaches psychoeducation classes
- Provides crisis intervention as needed
QUALIFICATIONS / REQUIREMENTS
- High School Diploma or equivalent required
- Case Management experience required
- Valid Driver’s License
- Ability to work a flexible schedule
- Strong organization and time-management skills
- Must be proficient in basic computer skills, including working with Microsoft products such as Excel, Word, and Outlook
- Experience working with electronic health records
