External Case Manager
We are seeking a dynamic Healthcare Liaison to join our team. The ideal candidate will have a strong background in healthcare, preferably as a RN or LPN, and possess established contacts within the healthcare community.
Responsibilities:
- Cultivate and maintain relationships with healthcare professionals, including physicians, nurses, and allied health professionals.
- Act as a liaison between our organization and external healthcare providers. Increasing census through cultivating relationships with multiple referral sources including hospitals, LTACHs, ALFs, ILs, physicians and the community at large .
- Identify opportunities for partnership and collaboration to enhance patient care and outcomes.
- Represent our organization at networking events, conferences, and community outreach programs.
Requirements:
- Current licensure as a Registered Nurse or LPN (preferred).
- Proven experience in healthcare, with a strong network of contacts.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Strong organizational skills and attention to detail.
- Must have the ability to network at all levels with strong drive, a passion for success, and a proven track record of census growth and development.
