The Healthcare Connection in Cincinnati is a National Health Service Corps (NHSC) approved site eligible for application to federal and state loan repayment and scholarship opportunities. We are seeking a Behavioral Health Provider , preferably bilingual Spanish / English, to provide brief, short term, solution-focused assessments and interventions geared toward functional restoration while consulting with our primary care providers to develop a clear treatment plan for the primary care team to manage.

The mission of The HealthCare Connection is to provide quality, culturally sensitive and accessible whole person-centered care, including primary health and basic oral services for the underserved, uninsured and underinsured of Northern Hamilton County. We are the first federally qualified health center (FQHC) in the state of Ohio and one of the earliest established in the entire United States. The HealthCare Connection teams with other agencies in order to meet the complete needs of patients and is proud to be one of five health centers nationally honored as “Models That Work” by the United States Department of Health and Human Services in 1995. In 2002, the Ohio Association of Community Health Centers gave THCC the Landmark Recognition Award for pioneering work in community health. After over fifty years of service, today we remain a leader in providing affordable, quality care to all ages and cultures, and have been recognized as a Patient Centered Medical Home (PCMH) through the NCQA.

What is the role of our Behavioral Health Provider?

• Effectively utilize a brief consultation model to communicate with the primary care provider. Use a team-based care approach that supports patients and their families in self-management, self–efficacy and behavior change.
• Provide consultation and training to other members of the primary care team in the area of mental health, chemical dependency, and behavioral medicine. Enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans.
• Consult with primary care providers / team to manage psychosocial aspects of chronic and acute diseases and manage patients with chronic mental medical problems efficiently and effectively. Working as a primary care team member to develop specific clinical pathways or best practice programs for targeted patient groups. Identify referral resources and facilitate referrals for further assessment and additional services as appropriate.
• Provide behavioral health services and substance abuse screening to children, adolescents, adults, couples and families to identify maladaptive coping strategies, and develop harm reduction strategies.
• Perform comprehensive bio-psychosocial assessment for mental health, substance abuse, co-occurring disorders, domestic violence and medical needs. Conduct follow-up as appropriate based on results of evaluation; medication adherence counseling and disease self-management counseling.
• Utilize electronic medical records to document all patient / caregiver interactions, as well as other electronic tracking tools and measures.
• Be immediately available to staff during working hours to assist in crisis Intervention as well as in a psychiatric emergency.
