The HealthCare Connection is currently seeking an Infectious Diseases Nurse Practitioner to provide medical care to adult patients at our Lincoln Heights and Mount Healthy locations.

We are a National Health Service Corps (NHSC) approved site eligible for application to federal and state loan repayment and scholarship opportunities.

We are the first federally qualified health center (FQHC) in the state of Ohio and one of the earliest established in the entire United States. Position Overview

The Infectious Diseases Nurse Practitioner (ID NP) will provide high-quality, patient-centered care for individuals with infectious diseases, including HIV, Hepatitis C, and other communicable conditions. The ID NP will integrate services with primary care, OB/GYN, Medication for Opioid Use Disorder (MOUD), and other programs to enhance patient outcomes and ensure a holistic approach to care. This role will ensure compliance with HRSA, Ryan White Program requirements, Ending the HIV Epidemic (EHE) funding guidelines, and other relevant regulations.

Essential Functions

Clinical Care & Patient Management

  • Diagnose, treat, and manage patients with infectious diseases, including HIV, Hepatitis C, and other communicable diseases.
  • Prescribe and manage antiretroviral therapy (ART), direct-acting antivirals (DAAs) for Hepatitis C, and prophylactic medications such as PrEP and PEP.
  • Perform routine screenings, immunizations, and preventive care for at-risk populations.
  • Provide patient education on disease prevention, treatment adherence, and harm reduction strategies.
  • Collaborate with primary care, OB/GYN, MOUD, and other teams to ensure integrated, whole-person care.
  • Support linkage-to-care efforts by working with case managers, care navigators, and community partners to ensure timely follow-up and retention in care.

Quality & Compliance

  • Ensure adherence to HRSA, Ryan White, EHE, and state/federal regulatory requirements.
  • Participate in quality improvement initiatives, clinical audits, and performance measures related to infectious diseases and linkage-to-care outcomes.
  • Maintain accurate and timely documentation in the electronic health record (EHR).
  • Contribute to the development of policies and procedures for infectious disease care within the organization.

Multidisciplinary Collaboration & Community Engagement

  • Work closely with behavioral health providers, social workers, and case managers to address social determinants of health.
  • Provide clinical guidance and education to medical assistants, nurses, and other staff on infectious disease management.
  • Engage in outreach efforts, including HIV/HCV screening events and health fairs.
  • Serve as a resource for staff on infectious disease-related topics and best practice.
  • Other duties as assigned.

Minimum Qualifications

  • Master’s or Doctorate in Nursing from an accredited Nurse Practitioner program.
  • Active Nurse Practitioner license in the state of Ohio.
  • Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or other relevant certification.
  • Minimum of 2 years of experience in infectious diseases, HIV care, or primary care with a focus on infectious diseases preferred.

Core Competencies

  • Ability to work collaboratively with primary care staff, patients and other behavioral health clinicians in a team-based integrated care setting.
  • Understand the relationship between medical and psychological aspects of health and disease.
  • Ability to apply the biopsychosocial model to the primary care setting and demonstrate a basic knowledge of pharmacotherapy.
  • Ability to apply population-based healthcare principles
  • Ability to perform focused assessment and rapid problem identification
  • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Ability to collaborate with medical providers in the treatment of psychosocial issues.
  • Excellent oral and written communication skills required.
  • Excellent problem-solving skills: will be able to match resources to the patient’s needs and assist the patient to resolve barriers in attaining their goals.
  • Must be able to handle confidential information.
  • Good organizational skills; able to coordinate multiple tasks as well.
  • Be available to work as scheduled, report to work on time and to work overtime if needed.
  • Be able to communicate clearly, think clearly, and concentrate on assigned tasks.

Preferred Qualifications

  • Strong clinical knowledge of HIV, Hepatitis C, STIs, and other infectious diseases.
  • Experience with Ryan White and EHE program requirements is a plus.
  • Familiarity with HRSA compliance and FQHC regulatory standards is a plus.
  • Excellent communication and teamwork skills to collaborate across service lines.
  • Commitment to health equity and serving medically underserved populations.
  • Population Health Management in the form of developing and implementing care gap programs and working with referral groups
  • Proven track record for building positive relationships with care team
  • Bilingual (English/Spanish) preferred, but not required.
  • Previous EMR experience; particularly with Nextgen
