Position Summary : The LPN Charge Nurse will be responsible to provide, coordinate and oversee the direct care services for our assisted living residents. This includes following and ensuring that other members of the care team follow the care and services identified in each resident's Individualized Serve Plan. In addition, the LPN Charge Nurse provides direct assistance with medication administration as identified in the Service Plan and ordered by the resident's practitioner. The LPN Charge Nurse is responsible for documenting and communicating resident change in condition and may respond to urgent, or emergent needs of residents in both assisted living and independent living which may include providing care within their scope of practice, including summoning and / or facilitating emergency medical services, as indicated. The LPN charge nurse ensures the completion of all assigned tasks by the other members of the health care team. Key traits include empathy, compassion and the ability to communicate effectively with other team members, practitioners, residents and families. The LPN Charge Nurse may assist with onboarding and training of new team members and will act as a mentor for other members of the care team.This position reports directly to the Health & Wellness Director, or Executive Director of Assisted Living.Duties / Responsibilities :
- Draft resident assessments and Service Plan for RN review and approval. Assessments and Service Plan must be updated at least annually as needed.
- Ensures training, orientations and in-service education is provided to staff in accordance with current Assisted Living regulations.
- Assess physical, spiritual, and psychosocial needs of resident and coordinate with consultants as needed. Coordinate need for extras, such as home care, room service meals, MSW, etc.
- Reports all incidents, investigates and notifies appropriate individuals.
- Reports and investigates all incidents of abuse, suspected abuse, and negligence.
- Reviews MD orders / MD notes, therapy, social services, mental health notes for change in functions / ability.
- Assures accurate count of all narcotics by shift.
- Assures new orders are transcribed correctly and necessary medication is requested from pharmacy in timely manner.
- Will assist in other duties assigned by the Director of Assisted LivingSkills & Abilities : Education : Graduate of LPN nursing programExperience : 1 year of nursing / prefer experience in Assisted LivingComputer Skills : Ability to effectively utilize basic software and emailOther Requirements :
- Ability to communicate in English effectively with others.
- Current Washington State LPN license.
- Current basic life support (BLS).
