The Hebrew Home at Riverdale by RiverSpring Living, a world-renowned long-term care organization located in Riverdale – North Bronx, serves over 18,000 people every day, both in the greater New York community and on our beautiful 32-acre overlooking the Hudson River.
RiverSpring Living provides a full range of care solutions including nursing home care, managed long term care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.
RiverSpring Certified Home Health Agency provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work as well as home health aide services.
Key Responsibilities :
As the Intake / Outreach Manager, you'll oversee the screening, evaluation, and admission planning for our CHHA Program. This role involves managing the entire process from inquiry to admission acceptance and coordinating the scheduling of Nurse Care Managers and vendors for timely evaluations. You'll work closely with the Assistant Dir of Admin. Operations to establish new referral sources.
Qualifications :
- Current NYS RN License and registration
- Bachelor of Science in Nursing
- Minimum of 2 years of experience in assessing patients for home care safety and developing care plans
- Minimum of 1 year of supervisory experience in a hospital or home care agency
- Patient Review Instrument (PRI) assessment experience preferred
- Valid driver’s license and proficiency in MS Office required
