HJF is seeking a Home Visit Coordinator for The Department of Medicine at USUHS that requires Home Visit, Family and Patient Coordinator Services for the medical student home visit program. The incumbent coordinates medical student visits to homes of children with special health care needs or disabilities or adults with chronic medical conditions, which the students complete related assignment for, as part of their 1st/2nd year Foundation Module and possibly again their 3rd year Bench to Bedside and Beyond module medical education. The coordinator also coordinates patients and family members with special health care needs, disabilities or chronic medical conditions who may be brought to the school for educational sessions with the medical students.
This position will be in support of School of Medicine (SOM-MED).

Responsibilities:- Contact parents and adults in the database by phone to schedule them as host families for the medical students. Match students to their geographic or medical condition preference whenever possible.
- Educate and inform each host family to assure they understand the timeframe for their visit, who will be coming and the purpose of the visit.
- Assist in recruiting new families as home visit hosts. Intake new families via email and phone.
- Maintain family database information by notifying the office staff when new information about a host family becomes available or when information such as phone number or address changes.
- Participate in in-home orientations of new home visit hosts as time in the task order permits.
- Help coordinate with billeted staff, MDL, Gate Access, and USU Security directly for any on campus activities. Responds to follow-up emails with attendees.
- Update intake/orientation materials for Home Visits. Revise, email, and distribute orientation materials.
- Other duties as assigned.
Education and Experience
- High School Degree or GED required. Associates Degree in health education, health science or health management preferred.
- Minimum of 0-2 years experience required.
Required Knowledge, Skills and Abilities
- Ability to effectively communicate orally and in writing.
- Ability to interpret, adapt and apply written guidelines and procedures.
- Computer literacy with a working knowledge of database programs and spreadsheets. Proficiency in MS Word, Excel and Outlook.
- Handle multiple tasks.
- Understand the importance and sensitivity of the information being managed/maintained.
- Excellent communication and organizational skills.
- Ability to obtain and maintain a T1 / Public Trust background check and Common Access Card (CAC).
Work Environment
- This position will take place primarily in an office setting.
Physical Capabilities
- Ability to stand or sit at a computer for prolonged periods.
- The hourly pay range for this position is $20.99-$22.00. Actual hourly pay will be determined based on experience, education, etc.
