Job Summary:
We are seeking a full-time RN Case Manager to provide comprehensive case management services to our hospice patients. The ideal candidate will have a strong background in hospice care and case management, with excellent communication and interpersonal skills.
Responsibilities:
- Conduct initial and ongoing assessments of patients to identify needs and develop individualized plans of care
- Collaborate with the interdisciplinary team to coordinate care and services
- Provide education and support to patients, families, and caregivers
- Manage symptoms, pain, and stress to promote comfort and quality of life
- Participate in on-call rotation (1 week per month)
- Maintain accurate and timely documentation of patient care
Requirements:
- Current RN license in the state of New Jersey
- Minimum 2 years of experience in hospice RN case management
- Strong knowledge of hospice regulations and guidelines
- Excellent communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of an interdisciplinary team
- Reliable transportation and willingness to travel to patient homes and facilities
