Job Description:
Provider, GUIDE Dementia Care Management Program
Position:
Advanced Practice Provider, Contract, 1099
Hours per week:
8-12 hours per week, with potential opportunity for increase as program census grows. Ability to choose scheduled days/hours. Must attend weekly IDT Wednesdays 8:30-9:30AM MST.
Rate:
APP: $60-67/hour
MD, DO: $85-92/hour
Role Summary:
The GUIDE Provider is an integral role to the success of The Holding Group’s GUIDE program, which serves Medicare beneficiaries living with dementia and their caregivers.
The provider role includes conducting cognitive-focused clinical assessments, providing symptom management and supportive care, and educating on disease symptoms and progression. The provider also provides dementia education and oversight of the dementia-focused care plans created and carried out by GUIDE Patient Navigators.
Responsibilities:
· Cognitive-focused evaluation including a pertinent history and examination
· Medical decision-making capacity assessments
· Dementia staging, using standardized instruments for staging of dementia
· Medication reconciliation and review, with a focus on high-risk medications for patients living with dementia
· Screening, or referral to screening, for hearing loss
· Making prescribing or deprescribing recommendations to the beneficiary's primary care or specialty care provider
· Understand and comply with CMS GUIDE program requirements
· Collaborate with physicians and other healthcare providers to develop and implement individualized treatment plans for patients
· Educate patients and their families about disease progression and symptom management
· Maintain accurate and up-to-date medical records
· Review and approve care plans completed by GUIDE patient navigators.
Requirements:
· NP, PA, MD or DO
· Current license to practice in Colorado
· Active DEA license in the state of Colorado
· Active malpractice insurance
· Strong clinical assessment and diagnostic skills
· Comprehensive knowledge on various dementia diagnosis including progression, treatment and supportive care
· Excellent communication and interpersonal skills
· Ability to work collaboratively and equally as part of a multidisciplinary team
· Proficiency in electronic medical record (EMR) systems
