The transition to adulthood can be difficult to navigate, especially for youth in their late teens or early 20s who have spent most of their lives in state care. The Home recognizes the unique issues that our youth "aging out" of the system face - including homelessness, unemployment, incarceration, early pregnancy and substance abuse-and offers resources and support designed to prepare them for their future as confident, self-sufficient adults in our community.

Our 25 community-based and residential programs serve thousands of at-risk youths from birth to age 26. Many of these children have experienced abuse, neglect, trauma, or a disrupted family life. As the oldest child welfare agency in America, we provide them with safe surroundings, permanent loving relationships, and a secure path toward a better, brighter future.

Program Description

This is an exciting opportunity to be a part of Metro Boston's first ever PACT-Y team through the Home for Little Wanderers. PACT (Program for Assertive Community Treatment) is an evidence-based home / community centered model supported by the Substance Abuse and Mental Health Services Administration (SAMHSA). The Department of Mental Health is launching PACT-Y for the first time for youth and young adults in the Metro Boston area. PACT-Y utilizes a highly integrated multidisciplinary team model that includes Clinicians, Outreach Counselors, Nursing, Occupational Therapy, Family Partners, Young Adult Peer Mentor, Vocational Specialists, and Psychiatrists / Nurse Prescribers to help support growth and recovery for youth and young adults in the community. Team members provide customized support to youth and young adults based on individual goals. This is your chance to be a part of this incredible new addition to the Massachusetts behavioral health landscape for youth and young adults!

How You Will Be Making a Difference

  • Collaborate to provide individual support, through ongoing routine contact to youth
  • Provide in-person mentoring services to youth both within their home communities and onsite
  • Participate as needed in quarterly treatment reviews and other meetings, as requested by the youth and / or program team
  • Facilitate peer groups with the goal of sharing personal strengths and interests that align with those of the program youth population
  • Partner with and advocate for youth through Restorative Practices
  • Assist program team in collecting and sharing information about community resources for our youth population
  • Partner with, engage, and retain resources identified to support program youth
  • Meet weekly for Supervision
  • Complete required documentation

Requirements

  • Personal experience living with significant mental health issues
  • Willingness to attend trainings prior to beginning job functions
  • Frequent walking, standing, climbing stairs, and traveling to client home communities
  • Computer literacy, including Microsoft Office and Internet Explorer with the ability to learn new software applications
  • Skills & Specifications

  • Excellent communication skills both verbal and written
  • Bilingual preferred
  • Completion of Core GIFT Training preferred
