The Homeplace is seeking a Licensed Practical Nurse to be our Wellness Nurse in our CBRF. Our Wellness Nurse is responsible for providing oversight of resident care and direct supervision to the med-passers and caregivers delivering care and services to the residents. The Wellness Nurse reports to the Community Director.
Schedule: The Wellness Nurse (LPN) is a full time non-exempt, position that will be scheduled weekdays and alternating weekends.

Responsibilities and Duties in coordination with the Community Director:
- Assist with resident assessments
- Participates in routine reviews of resident service plans to identify needs for additional assessment, health monitoring, or other coordination of care needs.
- Responds to resident or staff concerns in a timely and appropriate manner.
- Coordinates residents' health care in collaboration with outside health care agencies i.e.; pharmacy, physician, home health, hospice, etc.
- Coordinates and/or supervises appropriate documentation for all changes of condition.
- Assists with training staff on cares, medication passes and proper body mechanics
- Notifies physician and/or designated agent(s) of resident status/change as appropriate
- Coordinates and/or supervises appropriate documentation for all changes of condition.
- Performs weekly review of medication administration records.
- Communicate changes in service plans to staff as the need occurs.
- Assists with monthly medication cycle fills, verifying medications for each resident
Qualifications:
- Graduation from an accredited School of Nursing, with current LPN license
- At least 2 years’ experience with long-term care, assisted living, home health or hospital setting preferred
- Experience working with residents with Alzheimer’s or other related dementias
- Ability to handle multiple tasks
- Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to health and wellness resident
- Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications
- While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
- An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics

Pay: From $22.00 per hour
Expected hours: 32 – 40 per week
