What are the responsibilities and job description for the Behavioral Health Triage position at The Hope Clinic?
As a member of the Behavioral Health (BH) team, the Triage position works to assist current and potential clients locate and obtain services and assistance such as whole person care, food, basic needs, and general resource support. The BH Triage position coordinates general patient advocacy at Hope Clinic, ensuring that funded goals are met via either direct service or partner organization resources.
This is a part-time position anticipated to work 24 hours per week. Monday and Wednesdays required, plus 1-2 Saturday mornings per month.
Required Education and Experience :
BA preferred
3 years of human service work, including resource advocacy for underserved populations
Experience working with diverse and occasionally high-intensity clients
Required Competencies :
Ability to demonstrate Hope Clinic's mission and core values
Strong communication and client engagement skills
Experience working with difficult clients
Flexibility to work weekdays and Saturday hours as needed
Solid knowledge of community resources
Proficiency with computers and general office equipment
Ability to effectively function in and support the mission of an inter-denominational Christian environment
Primary Duties and Responsibilities :
Connect clients to internal and external services for whole person care, including spiritual care
Provide direct assistance to clients in obtaining necessary services for which they are eligible
Build partnerships with external organizations to effectively assist patients and develop respectful, reliable, and trusting relationships
Use various database, online, and paper systems to enroll patients in needed services and maintain patient records
Provide direct client contact and support, engaging clients and answering questions, in person and / or by telephone.
Make patient appointments and referrals to other departments or community agencies
Provide training and consultation to First Impressions Teams on BH services and front-line communication
Coordinate client triage and referral for resources and / or BH services
Develop processes to provide patient access to the food pantry, nutrition education, and related areas of assistance as needed
Assist the Medication Assistance Program Coordinator in identifying options for free or low-cost medications and track enrollment status; assist patients with completing appropriate applications for medication assistance
Teach capacity-building skills to clients to complete forms, anticipate re-enrollment deadlines, and utilize resources appropriately
Help clients identify and remove barriers to care and community resources, working with the Hope Clinic Behavioral Health staff as necessary
Maintain accurate, organized, and confidential patient advocacy records, observing all required HIPAA, privacy, and confidentiality regulations
Attend BH Staff meetings to develop common service delivery and reporting tools and enhance collaboration and communication between Hope Clinic and other medical services
