JOB SUMMARY:
Responsible for providing comprehensive education and support to individuals and their families with diabetes, obesity, and any metabolic/endocrine associated disorder, helping them to manage their condition and achieve their health goals.
GENERAL REQUIREMENTS:
- Conduct individual and group diabetes education sessions
- Develop personalized care plans for patients with diabetes
- Provide instruction on self-monitoring of blood glucose levels
- Teach patients how to administer insulin and other diabetes medications
- Offer nutritional counseling and meal planning guidance
- Educate patients about the importance of physical activity and exercise
- Assess patients' knowledge and skills related to diabetes management
- Support patients in setting and achieving diabetes-related goals
- Monitor and document patients' progress over time
- Provide timely follow up to patient questions, phone calls, etc.
- Timely documentation in the patient’s medical record to ensure timely coding of services provided to optimize patient safety and assure compliant billing practices are followed.
- Collaborate with other healthcare professionals to ensure comprehensive care
- Stay up-to-date with the latest diabetes management guidelines and research
- Conduct community outreach and education programs
- Advocate for patients' needs within the healthcare system
- Participate in professional development and continuing education activities
GENERAL SKILLS:
- Ability to communicate in English, both verbally and in writing.
- Additional languages preferred.
- Experience working with diverse patient populations
- Strong communication and interpersonal skills
- Proficiency in electronic health record systems
WORKING CONDITIONS:
- General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.
- May be exposed to high noise levels and bright lights.
- May be exposed to limited hazardous substances or body fluids, or infectious organisms.
- Periods of high stress and fluctuating workloads may occur.
- May be scheduled as needed including overtime.

JOB SPECIFIC COMPETENCIES:
- Reviews, revises, and participates in the development of the facility’s patient care policies to ensure the inclusion and support of nutritional care.
- Assures that quality procedures are appropriate for collecting and summarizing.
- Recommends suitable clinical and administrative references pertaining to nutrition.
- Collaborates with physicians, mid-levels, and clinical staff to identify and provide appropriate nutritional counseling.
- Establishes treatment plans and communicates with physicians/mid-levels regarding nutrient needs.
- Meets directly with patients to identify educational needs, provide individual instruction, utilize informational handouts/booklets, or refer to community resources as needed.
- Establish priorities, goals and objectives which meet nutritional needs and are consistent with available resources.
- Responsible for corrective action on any deficiencies or recommendations from health agencies. i.e.: Division of Human Resources, DNC, etc.
- Will serve as a liaison to industry representatives to assure that the clinic is properly equipped and utilizing technology inherent in the treatment of patients with diabetes or other applicable conditions.
- May participate in outreach activities as defined through strategic growth initiatives for the practice.
- Perform other duties as assigned. Must be HIPAA compliant.

Qualifications
EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:
- Active Registered Dietitian (RD) in Georgia
- B.S. Degree in Foods and Nutrition or Advanced degree in nursing, nutrition, or a related field.
- Successful completion of Dietetic Training Program with six months experience under a registered dietitian. Preferred 2 years of professional general practice experience.
- Eligible for registration with the Academy of Nutrition and Dietetics.
- Experienced and knowledgeable in clinical and administrative dietetics.
- Effective writing and speaking skills.
- Or Active Registered Nurse (RN) License in Georgia (includes nurse practitioners and clinical nurse specialists)
- Certified Diabetes Educator (CDE) certification preferred. The Hospital Authority of Miller County will assist you in working towards this certification if RN does not hold current CDE certification.
