What are the responsibilities and job description for the Foster Care Intake Specialist position at The House of The Good Shepherd?
Basic Function/Overall Responsibilities: Overall management of intake and admission process for the Foster Care program. Ensure timely and appropriate communication regarding admissions, transfers, and discharges. Responsible for inputting information into Electronic Health Record for accurate billing.
Principal Assigned Responsibilities:
- Review all referrals sent to Foster Care and work closely with the Assistant Director of Home Finding and the Director of Foster Care to make determinations around acceptances and admission of children into Foster Care.
- Communicate with referring workers to gather additional information on referrals so an informed decision can be made about level of care and best placement match.
- Work closely with the Home Finding team on referrals and intakes.
- Coordinate all admissions and transfers within Foster Care. Enter all admissions and discharges into the Electronic Health Record. Complete and distribute SSPS notifications for admissions, transfers, discharges and respites.
- Maintain regular communication with County Workers regarding the status of referrals.
- Complete 2999 forms and ESSA referrals for admissions and discharges and participate in ESS meetings.
- Participate in weekly integration meeting and update meeting notes.
- Organize intake and transfer meetings, take notes at meetings and distribute to the team for any follow-up needed.
- Manage the Vacancy List spreadsheet that lists all foster homes and placements. Monitor this list for accuracy with home numbers and vacancies.
- Monitor and edit the kinship and certified foster parent distribution email groups.
- Communicate with the agency business office to ensure accurate billing occurs.
- Manage the clothing vouchers and gift cards issued to foster children.
- Prepare weekly reports and send to the county. These include but are not limited to integration notes, census, school locations and vacancy list.
- Assess trends in referrals and provide feedback for agency use in planning and program development.
- Assist the AD of Home Finding with audit, KinGap and adoption requests by preparing and sending needed documentation to QI, the county or OCFS.
- Other related tasks as directed by supervisor.
Relationships-Supervision and Coordination: The Foster Care Intake Specialist is under the direct supervision of the Assistant Director of Home Finding and has regularly scheduled supervision with that individual. They must maintain productive working relationships with the Director and Assistant Directors of Foster Care, as well as all members of the Home Finding team. They must also maintain working relationships with key individuals from referring agencies.
Education and Experience Requirements:
- Minimum bachelors degree in human services field or related field.
- One year of relevant experience required. Prior training experience preferred.
