With the goal of access, quality, efficiencies, and patient satisfaction, the Licensed Clinical Therapist - Clinical Supervisor provides culturally sensitive mental health services to Hurtt Family Health patients through an integrated and comprehensive team-based model. Provides assessments, referrals, and follow-up appointments that include developing and implementing therapeutic treatment plans with clients in a strength-based model in all clinical settings. Participates in the development of other therapy team members through direct clinical supervision and indirect support and clinical expertise consultation. Ensures compliance with policies and procedures of the overall program and timely completion of respective reporting requirements. Collaborates with Director on the ongoing development of Behavioral Health services at HFHC.

 

What You'll Do: 

Job Responsibilities & Duties

  • Projects and carries out the goals of the clinic to provide high quality integrated care that is compassionate, culturally sensitive, equitable, and trauma-informed, to all patients and families who are eligible to receive services at HFHC.
  • Works efficiently with care team members to provide care to an average of 6 patients per 8-hour work day and responds to assessing the need for behavioral health services for all walk-ins while on duty.
  • Provides direct behavioral health clinical therapeutic and integrated medical services to individuals, families, groups and communities to all HFHC service areas.
  • Assesses client’s strengths, needs, abilities, and preferences and formulate a diagnostic impression based on current DSM criteria. Performs biopsychosocial assessments and develops collaborative treatment plans for all ongoing clients. Determines therapeutic approach based off clients’ needs and history of trauma (i.e. treatment frequency and duration of therapy).
  • Discusses progress toward goals with client and works to meet client needs effectively and resolve individual barriers through patient follow up.
  • Actively participates in the integrated care team setting by consulting and collaborating with all other service departments and staff including but not limited to Primary Care, Addiction Medicine, etc.
  • Maintains accurate and timely clinical documentation consistent with agency standards. Scans all paperwork pertaining to patients into the EMR.  Ensures patient and provider have signed all appropriate paperwork when applicable.  Documents progress/case notes in the EMR that include diagnostic mental health assessments, treatment plans, and progress notes in addition to other reporting and statistical data in a timely manner as required by organizational and legal guidelines.
  • Appropriately utilizes the electronic health record, to include chart documentation and coding for services provided and timely encounter closure within 48 hours. Documents all patient encounters thoroughly in the EMR system to meet standards of practice and all required UDS, CQMs and HEDIS measures.
  • Provides ongoing training for through direct clinical supervision, indirect support, and clinical expertise consultation to increase all team members’ effectiveness and base of knowledge.
  • Provides supervision and consultation of Associate clinicians and or students within scope and as required by the BBS and law.
  • Develops the Behavioral Health department in partnership with department management.
  • Provides new hire training and orientation as needed.
  • Maintains good working relationship and communication with all staff, reflecting professionalism in all interactions.
  • Works closely and collaborates with other Behavioral Health and HFHC staff to provide the best possible care within scope of practice and services. Consults with relevant care team members through case discussions and presentation of pertinent client information.
  • Maintains a clean, safe and unobstructed workplace.
  • Attends All Staff meetings, Provider meetings, and other meetings as assigned; and is responsible for all information presented.
  • Provides crisis intervention counseling as needed and outreach counseling to community settings such as schools, home visits, and other settings as assigned.
  • Provides specialized counseling groups or classes within the scope of license and training.
  • Provides community mental health presentations and PowerPoint presentations on behavioral health preventative measures to the community and/or clinic staff as directed.
  • Works cooperatively with outside community resources and other in-house services (i.e. medical, dental, etc.) or support programs to supplement in office treatment and counseling to coordinate services for clients. Develops community relationships for patient referrals, representing the values of HFHC in a compassionate and professional manner.
  • Assists in development of departmental policy, procedures, and quality improvement activities within the clinic as directed.
  • Participates in ensuring quality care delivery through enhancing patient experience, improving clinical experience, improving population health, and reducing costs.
  • Lead patient access and efficiency by modeling high effectiveness, training, and assisting other providers in maximizing time and access to care. This includes providing feedback to admin and provider mentorship.
  • Ensures timely credentialing by maintaining LCSW/LPCC License, NPI Number, CEU, PAVES, PECOs, etc. and submitting proof annually. Ensures timely communication with HR and administrative staff to maintain credentialing and audit compliance requirements.
  • Adheres to accreditation and compliance standards/guidelines and gets yearly culturally supportive training.
  • Complies with Hurtt policies and procedures and regulatory Federal, State and local rules and regulations including public health advisories and CMR reporting.
  • Participates in all safety programs which may include assignments to an emergency response team for Disaster Preparedness.
  • Participates in Peer review, Hurtt QI programs, and Continuous Quality Improvement, and in assuring quality patient care is maintained.
  • Reports and responds to complaints regarding clinical care, resolving any complaint with expediency and in a professional manner in coordination with staff.
  • Performs any and all such duties that are within the scope of this individual’s certifications that are requested by HFHC providers.
  • Performs other job duties as requested by the Behavioral Health Director.
