- Hours:Monday - Friday | One Half-Day Saturday per Month | 10 Hour Shifts 4 Days per Week or 8 Hour Shifts 5 Days per Week

- Call Schedule: Phone Only | Rotated Weekly Every 7 Weeks

- Compensation: Salary based on experience with additional bonuses and incentives-Sign-on bonus -Up to $12,000 earned productivity bonus- Relocation assistance provided for FTE!

We would love the opportunity for you to be on our team!DETAILS:- This position will work and spend time in our 2 locations: Hazel Crest and Chicago Heights, about a 10-minute distance between the 2 locations- Provide routine primary care as guided by policies and procedures along with the supervising Physician- Provide Family Planning services as requested according to protocol- Provide prenatal services as requested according to protocol-Provide care to both a pediatric and adult patient panelQUALIFICATIONS:- IL License REQUIRED-2 year experience as an NP in outpatient, primary care preferred. This role will not accept new graduates, as they must be able to commit to a level of autonomy
