Qualifications
- Works collaboratively with physicians and other health care personnel in patient evaluations and treatment to further their understanding of significant social and environmental factors underlying patients health problems.
- Provides direct case management services to assist patients in receiving necessary resources and services such as: Medicaid, Child Health Plus, Welfare, DAS, Social Security, Child Care, Home Care, Housing/Shelter/Safe Homes, Substance Abuse Treatment, Mental Health Treatment, HIV Services, Legal, other Health Insurance etc.
- Completes sliding scale, 340 B pharmacy benefits, financial waiver, and cancer service applications as needed.
- Helps assist patients with insurance options including pooled trusts.
- Documents all patient services in patients electronic medical record, and closes encounters within 24 hours.
- Prepares case management assessments, patient histories, service plan and reports.
