Description
The QIDP will coordinate personalized plans, monitor progress, and ensure compliance with all regulatory documentation requirements. Develop and implement person-centered documentation, including but not limited to Staff Action Plans, Individual Plan of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, Levels of Supervision, and Monthly Summaries. Lead interdisciplinary team (IDT) meetings and ensure quality active treatment programs. Maintain accurate electronic records and ensure compliance with OPWDD regulations. Provide input for individualized training programs and progress monitoring. Support individuals in achieving independence in life skills and social interactions.
Qualifications
BA degree in a Human Services field with 1 year of relevant experience is preferred. Valid New York State driver’s license. Proficiency in person-centered planning, instructional methods, and progress tracking is essential
