What You’ll Do:
• 📋 Coordinate Personalized Plans: Develop and implement individualized plans for each client, including Staff Action Plans, Individual Plans of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, and Levels of Supervision.
• 🤝 Lead Interdisciplinary Teams (IDT): Facilitate team meetings to ensure that clients receive quality active treatment and that their needs are met holistically.
• 📅 Monitor Progress: Track and evaluate the progress of clients’ goals and achievements, adjusting plans as necessary.
• 📝 Ensure Compliance: Maintain accurate, up-to-date electronic records and ensure all documentation is in compliance with OPWDD (Office for People With Developmental Disabilities) regulations.
• 👥 Support Independence: Assist clients in gaining independence with life skills, social interactions, and other personal goals.
• 📚 Provide Input for Training Programs: Help develop and implement training programs for staff and clients, monitoring progress and outcomes.
What We’re Looking For:
• ✅ Education: Bachelor’s degree in a Human Services field (or related field) required.
• ✅ Experience: 1 years of relevant experience in a similar role preferred.
• ✅ Skills: Proficiency in person-centered planning, instructional methods, and progress tracking. Strong organizational, communication, and interpersonal skills.
• ✅ License: Valid New York State driver’s license is required.
• ✅ Knowledge: Understanding of OPWDD regulations and the ability to maintain accurate documentation.
Job Details:
📍 Primary Location: 32 Warren Avenue, Tarrytown, NY
🕒 Schedule: Monday–Friday (Full-Time, 40 hrs/week); flexibility required for occasional evenings and weekends.
🚗 Travel Required: Must be willing to travel between IAHD programs in Westchester and the Bronx.
