The Iowa Clinic is seeking a Care manager for our West Des Moines location. A Care Manager collaborates with all members of the multi-disciplinary healthcare team to ensure the delivery of high-quality, cost-effective care, as evidenced by outcomes of care management of chronic diseases, coordinated transitions of care; achievement of select quality outcome metrics; appropriate utilization of health management programs and resources, within the structure of a patient-centered medical home.

A day in the life…
Wondering what a day in the life of a Care Manager at The Iowa Clinic might look like?

High Risk Patient Management:
- Identify patients with chronic illnesses that would benefit from care management.
- Outreach to patients to discuss care management; assess for patient interest/engagement
- Collaborate with the provider, healthcare team, and patient/family to evaluate health status, promote education, identify barriers and develop patient specific care plans with SMART goals. Promote patient self-care and assist patient in securing health care and community resources. , Ensure care plan consistency across providers
- Communicate with patients regularly to assist on disease related symptom management, address compliance with care plans, promote lifestyle choices to improve prognosis.
- Verify chronic disease conditions are listed accurately in EMR, enter chronic disease conditions accurately in EMR.

Assist Transitions Of Care nurse with Transitions of Care Process: (as needed):
- Provide follow up on inpatient admissions and emergency department vists; working collaboratively with the provider and healthcare team to establish plan of care post discharge, including follow up appointment in the clinic to prevent unnecessary readmissions/visits.
- Encourage and assist with closing patient health maintenance quality measure gaps to ensure the highest quality of patient care. Measured by patient outcomes and payor incentives.
- Collaborates with providers, clinic staff and clinic leadership to identify patient care needs and assist with resolving day to day issues.
- Maintains awareness of own safety in carrying out duties and responsibilities. Adheres to universal precautions and infection control policies and procedures.
- Assumes accountability for enhancing own knowledge through professional associations, journals, workshops, in-services, etc. Keeps professional license current.
- Maintains patient confidentiality and uses discretion in patient interactions.
- Promotes a positive, professional image of The Iowa Clinic both internally and externally. Promotes positive interpersonal relations.

Education:
- RN with strong clinical background required
Licensure/Certification:
- Current State of Iowa Nursing License
- CPR certified or complete certification within first 3 months of employment
Previous Work Experience:
- Two years clinical experience required, adult specialty-specific experience preferred. Experience in chronic disease management or health coaching strongly preferred.
